Insulin Pump & CGM Education for Optimal Insulin Use in Diabetes

This course consists of three 6-hour sessions over 3-5 weeks and is intended as a joint learning event for people with diabetes, their family members and Health Care Professionals. The course has been accredited by RCN and the accreditation has also been applied from the RCP/ABCD and BDA for doctors, nurses and dietitians respectively.

 

Day 1 - Starting Insulin Pump Treatment

  • Introduction to pump therapy, benefits & challenges
  • Basic education & competences necessary before pump therapy
  • Knowledge of available pumps and their distinguishing features - Personal preferences
  • o Demonstration of Animas 2020 pump (Animas)
  • Education at the start of pump therapy: Two days
  • o Demonstration of Paradigm Veo pump (Medtronic)
  • More on setting up a pump - changing cartridges, priming the pump, programming basal rates and standard meal boluses, infusion sets and accessories
  • o Demonstration of Accu-Chek Combo pump (Roche)

Day 2 - Education 1 and 3 Months After Pump Start

  • Two Half Days: Reviewing original goals and what has been achieved; problem-solving & fine-tuning of Basal Rates, Insulin to Carb Ratios & Correction Factors
  • o Demonstration of DANA R pump (Advanced Therapeutics UK)
  • Future of Insulin Pumps: Tubeless, Minipumps & Patch Pumps - Cellnovo, OmniPod, Solo, Flexi...
  • Downloading pump data: analysing and understanding appropriate actions needed
  • o Demonstration of CareLink, Animas eZManager, SmartPix and Diasend Downloading
  • Lightening lows and hitting highs with pump therapy --Managing exercise, holidays, travel, illness, pregnancy, puberty, feasting & fasting
  • Enhancing pump therapy with Continuous Glucose Monitoring

Day 3 - Continuous Glucose Monitoring (CGM) in Real Life - Let Glucose Be Your Guide

  • Continuous Glucose Monitoring vs. Blood Glucose Testing
  • Understanding CGM and overcoming challenges & obstacles
  • § Practical demonstration of Navigator 1.5 CGM system (Abbott)
  • What we can learn from using CGM real-time readings - Case studies from real life CGM records
  • Understanding the ‘Lag Effect': physiological lag between capillary blood and interstitial glucose
  • § Practical demonstration of Paradigm Real-Time MiniLink (Medtronic)
  • Downloading CGM data and finding trends
  • § Practical demonstration of DexCom Seven Plus CGM system (Advanced Therapeutics UK)
  • Effective trouble shooting in CGM - Workshop with CGM reports

Advanced Carbohydrate Counting & Insulin Dose Adjustment Course

This course consists of three 4-hour sessions over 3-4 weeks and is intended as a joint learning event for people with diabetes, their family members and Health Care Professionals.  Accreditation has been applied for this course from the RCP/ABCD, RCN, and BDA for doctors, nurses and dietitians respectively.

 

  • § Practical hands-on learning with foods you eat
  • § Course takes place over three weeks to allow self-learning at home to consolidate what has been covered
  • § Opportunity to hire a Continuous Glucose Monitor (Navigator) for 5 days to observe the effect of carbs and verify insulin dose accuracy - needs to be booked in advance

How to calculate carbohydrates - Tools in practice: Foods with carbohydrate, NutriScales practice, Reference books, Food labels, Meal plates and carb learning websites

Keeping accurate records

Continuous glucose monitoring - How can technology help?

 

Session 2:

 

Grams of carbs in the foods you brought with you

Formula for meal bolus dosages: 450/500 Rule - Carbohydrate to Insulin Ratio

Formula for correction bolus for off-target glucose levels: 85/100 Rule

Blood glucose targets & Duration of insulin action

 

Session 3:

Seeing the bigger picture: Total daily dose - Basal dose - Meal boluses - Correction doses

Fasting testing for Basal insulin verification

Does it work? Verifying your insulin bolus accuracy: Testing your meal boluses and correction doses.

 

What will you gain after completion of this course?

After completion of the course, you should be able to work out the amount of carbohydrate in your food; begin to apply your carb ratios and correction factors to decide on your insulin dose; to have built up an almost-complete carb list of your typical meals. You should be able to verify if / when / how these figures need readjusting.
