Diabetes Connect
Diabetes Connect is a telephone and e-mail service supporting patients with diabetes.
As a Diabetes Connect member, you will receive:
- Answers to any queries you may have regarding your diabetes
- Reassurance that you can communicate with your diabetes team from home and abroad
- Fast feedback and answers to your queries via text message, e-mail or telephone
- Follow-up support immediately following a change in regimen
- Improvement in well being and quality of life
Following your consultation with our Senior Diabetes Nurse you will be entitled to 6 e-mails or phone calls as part of your consultation follow-up support. If you would like to continue this support or require further help throughout the period before your next appointment, you may purchase one of the following packages:
- 6 e-mails or phone calls - £50
- 12 e-mails or phone calls - £80
